Enrollment & Eligibility

The Upper Valley Adult Day Care provides adult day services for older adults (age 60+) and individuals with physical or cognitive impairments. We offer person-centered care that supports independence and helps participants remain in their homes and communities.

Who We Serve

Enrollment is approved on a case-by-case basis. You or your loved one may qualify if any of the following apply:

  • Lives alone or with a caregiver

  • Caregiver needs daytime respite

  • At risk of nursing home or residential placement

  • Has cognitive or physical challenges

  • Can engage in a group setting and contribute to a positive social environment

Please note: All tours must be scheduled in advance.

You can also begin the enrollment process by completing our online forms. Once we receive your completed paperwork and medical records from your healthcare provider, our Program Manager will contact you to finalize the enrollment and discuss next steps.

  • A Social Security Award Letter

Payment & Financial Assistance

Payment Options

  • Private Pay – $26.28/hour (all-inclusive)

  • VT Medicaid – Includes Long-Term Care (LTC) and Moderate Needs programs

Grants & Financial Assistance

  • Jog Your Memory 5k (Up to $3,500/year. Must have Alzheimer specific diagnosis)

  • Senior Solutions Dementia Grant ($1,000–$1,500/year)

  • Senior Solutions Respite Grant

  • APDA Grant ($500/year for Parkinson’s care)

  • Ottauquechee Health Foundation (various supports available)

Sliding Fee Scale

  • $21.02/hour

  • $23.65/hour

  • $26.28/hour
